We Welcome You
NBIH welcomes you and your family! Our intake process is the first step in accessing services through the hub to ensure we can smoothly connect you with the most appropriate clinician for you or your family member’s needs. As a patient with NBIH, you will have access to interprofessional healthcare providers in collaboration with our traditional health and healing team. This will allow for comprehensive and client/patient-centered care to ensure that your health and healing are prioritized. As a patient with the NBIH, you will be welcomed into a circle of care. Within this circle, you have the right to culturally safe and culturally informed services that are intrinsic in supporting your health and wellness on a physical, mental, emotional and spiritual level. When you contact us, we will ensure that your immediate needs are met first. We will then provide you with a copy of our intake form and schedule an initial intake appointment to meet with you. Your first visit to the NBIH will include an intake appointment to discuss your current health status. From there, we will collect other information to assist with the most appropriate pathway of care for you and/or your family. You may be asked for your proof of lineage to be eligible for service.
What to expect for your intake appointment
Please bring your health card, proof of Indigenous lineage (e.g., status card, Métis/Inuit registration) and a list or print-out of the medications you are currently taking. Please be sure to indicate any traditional medicines, supplements, and over the-counter medications. It would be helpful for you to bring the completed copy of the intake form as well. If you don’t have it, don’t worry, we can review together and discuss any concerns you may have.
The intake appointment could last up until one hour, depending on your needs. We will use this time to learn more about you and your story so that the best service(s) we have to offer can be provided to you.
We recognize that our intake form is comprehensive. We respect your privacy and ensure that our process is delivered in a culturally safe and confidential manner. The Staff at NBIH will meet all obligations related to your privacy rights. Any information collected on the intake form and/or at the intake appointment helps us to provide you with the best care possible and assists us in our advocacy work for current and future possibilities. While appreciated, please do not feel obligated to answer all of the questions on the intake form.
There is always the option to skip the question or to select “prefer not to answer.” It is essential for you to know that should you choose not to answer a question, you will still receive the same culturally safe, high-quality care from our hub. We are accepting Intakes!

The North Bay Indigenous Hub receives funding from the Ministry of Health and Long-Term Care’s Primary Health Branch to support the Indigenous community at the clinic. Please note that this funding is specifically given to support the Indigenous community ONLY (First Nation, Metis and Inuit). If you are submitting an intake for consideration at the North Bay Indigenous Hub, please note that we have the right to request proof of ancestry (i.e. membership of First Nation or Metis Nation). This is to mitigate the risk of accepting intakes with false or exaggerated connection to Indigenous identity and service ONLY those who meet the criteria.
